What you need:

I used superimpose.

1. Open the paper as your background and the one as your foreground then go to transform and make the filter multiply if you want the png to look like it's crumpled too or leave it on normal. Merge and save.

Or you can take a picture and make it crumbled just like the png one.

1. Open your photo and then open the crumbled paper as the foreground. Go to transform and make the filter multiply. Merge and save.

Add your text and your done.
