Imagine person A of your OTP is a surgeon, and a really good one at that. 

Now imagine your OTP got themselves in a situation where they can't/don't have access to a hospital and A needs to be operated on.

 A comes up with the idea that B will perform surgery on them with whatever tools they have, while A tells them what to do. 

B fails and A dies at the hands of B.
