Kato has an extremely muscular, large physique, and very tall build. In Yakuza 0, he has a black slicked back hair and clean shaven. Throughout Yakuza/Yakuza Kiwami onwards, Kato spiked his hair bangs and he has full beard.

Tattoo:
Kato tattoo depicts 2 different demons. The Samurai demon on his back, and a demon on his left arm. The samurai demon is a reference of his loyalty towards his patriarch, and he will fight for him even if it means forfeiting his own life.

The demon on the left arm symbolizes his anger and ambitions, hid in the cover of silent and loyal subject, biding it's time to strike.

Attire:
In Yakuza 0, Kato is seen wearing white shirt over black vest, grey coat, grey pants, and black sneakers. He also usually wears black rimmed glasses to hide his death stare.

From Yakuza/Yakuza Kiwami onwards, he wear his trademark suit consists of a black hoodie under black trench coat, grey pinstripe pants, and black sneakers. He wear a diamond watch, 6 diamond rings, 3 on the left hand fingers and 3 on right hand fingers, and he still wears his black rimmed glasses.

The other time he wear something formal is during funeral ceremony, where he wears black suit with dark grey shirt underneath, a matching black pants, a black tie, black sneakers, and black gloves.

Personality:
Kato is a silent and honor bound yakuza, with extreme loyalty towards the clan and family he serves. Although from young age, he's mostly reserved, silent, and quiet, to the point he is mocked by his peers as a silent brat and a nerd for his choice of clothing.

One that makes Kato different is his passive and pessimistic attitude towards everything, and this attitude is even more present especially during meetings, where he usually sat down with his left leg usually nestled on the table, leaning on his left hand, and closing his eyes, as if he's asleep.

Although mostly seen passive and poker faced, he has a burning ambition to take, monopolize, and lead, and such he usually employs either underhanded tactics such as kidnapping hostages or divide and conquer, and if situations forces to, he employs an all out war tactics.

However, as passive as he is, he is not the most patient man. He has a short fuse, and when provoke, he will immediately throw the first punches, and will not going to stop until they beg for mercy, not even if authorities comes to pull him away.

Kato is also very strategic when it comes to planning, and very much perfectionist. He won't take no for answers, and if any one of the family members didn't want to follow them, he will either beat them to the point of critical, or straight up killing them.

Kato is also a battle maniac. Even during his time at school, he often goes to the Underground Colosseum to watch and sometimes participate in a fight.

Fighting styles:
Don't be fooled by his passive attitude, as Kato is a very powerful giant, and will fight without mercy. His attacks are powerful, and surprisingly fast too. He's also agile for a man that's big, as he can jump over a person and parkour around some areas.

Yakuza 2:
Kato's main combo is a 4 hit combo: right hook, left hook, right uppercut, and left spinning kick. He sometimes does:
- headbutt into right backhand.
- jumping knee into right elbow.
His down attack is a jumping stomp.

Yakuza 4 & 5 (Challenge mode):
Kato's moveset is his Yakuza 2 moveset with new moves added:
- a throw, where he grab his opponent by their neck and smack them with a flashbang.
- lunging Superman punch
- a combo of left body hook, right body hook, and knee to the groin, that could stun.
- right straight, right straight, and right haymaker.

Yakuza 0:
Kato can utilize different fighting style according to how he perceived the battle. He can change between a slow but destructive power of his size with his wrestling (Strongman), fast and skillful with Taijutsu (Ninja), or accurate and deadly with kickboxing and Revolver combination (Marksman).
- Strongman:
Strongman refers to an exhibitor of strength that usually performs a feat of strength. Or in this case, the way Izanagi fight by emphasizing his large physique and strength to brutalize his opponent.

Strongman style has a slow but powerful moves, emphasis on professional wrestling, street fighting, and crude kicking techniques. He has a defensive wrestling stance, with him hunched and in low stance, left and right arm reaching out, with his left hand reach out the farther than his right hand. In this style, he has a dark grey aura. His aura color is indicating of Limbo, the gates of Hell.

His rush combo is left hand chop, right shin stomp, another right shin stomp, and left ruffian kick. Other combos included;
- right hook, left hook, right uppercut, and left spinning heel kick.
- double hammer fist, right spinning heel kick, and left shoulder tackle.
- Mongolian chop, right big boot, right spinning back fist, and jumping dropkick.
- headbutt, right hand chop, and discus lariat.

He also has other moves, consist of:
- right front kick, into a tackle, which if connect, Izanagi will lift his opponent in fireman's carry position and does a death Valley driver.
- a 3 hit combo, consist of right haymaker, left knee, and jumping right knee.
- left shin stomp, into a DDT.
- jumping crossbody.
- flying elbow smash, and if connect, he will do a kip up and a jumping elbow drop.
- a rugby tackle, before mounting and start ground and pound.
- his downed attack is a double foot stomp.

He has 2 different grab depending on his health:
- if he's not in Heat mode, he'll do a telegraphed grab, where he grab his opponent by the face and does a claw slam.
- if he's in Heat mode however, he'll have a quick grab that's inescapable, where he does a a powerful gut punch, before forcing them to kneel and deliver a vicious curb stomp.
