Here is the form & the requirements you need to know about in order to get a cover:

*Please tag at least 3 people

-Do not PM the form or I will not make the cover.

-You must fill out the form ALL AT ONCE in the comments below. Do not leave several comments.

-If you are not satisfied with your cover please let me know. I will try to make a second one that you like.

-Do not say 'surprise me' or 'whatever you think looks best' because I will not take that. Be specific.

-Even if you aren't fully satisfied with the cover I've made, you have to complete the payment. You need to know that I take time out of my day just to do this for you.

-I don't do covers for LGBT stories

-I will PM you the payment once your cover is finished (:

-Give me credit on your book's summary.

-Tag 3 people you know!

IMPORTANT THINGS TO KNOW

{I may end up making you up to 3 covers depending on what you want, genre, etc. However, not everyone will get more than one. Do not yell at me if you only get one just because you saw someone else get two or three.}

{If you don't like something on your cover please tell me. I will change whatever it is you don't like. However, you will only get one more chance. After that, I will not remake your cover again.}

***Delivery***

{Covers are usually delivered within a day. However, if I get a lot of requests, I do ask you to be patient. It usually won't take me more than a day to get to you, though.}
